The best way to do that right now is to embed a Google Form into your InDesign document.

  1. Open a form in Google Forms.
  2. In the top right, click Send.
  3. At the top of the window, click Embed Embed.
  4. To copy the HTML that appears, click Copy or press Ctrl + c (Windows) or ⌘ + c (Mac) on your keyboard.
  5. Paste the HTML into your InDesign document directly, or use Object > Insert HTML to insert the embed code into your InDesign document.


InDesign will then render a preview of the HTML*, and it will export with in5.


*At the time of writing this InDesign's HTML preview is buggy, choosing Object > Edit HTML, then clicking OK without making changes often fixes the issue. Either way, even if the preview is incorrect and the size of the frame is correct, in5 will export it as expected.


Your form will be live within the page, so it can be submitted.
